Should the dot on the cam pulley be straight up when the timing marks on the crankshaft pulley line up with the mark on the lower timing belt cover?

Does the cam pulley locate on the camshaft with a key or something similiar to the cranshaft gear?

yes
theres a little guiding notch on the end of the camshaft, and a little hole on the cam gear, so theres only one way it can fit on. and the cam gear dot should be pointing straight up in regards to the engine angle, which would be slightly to the passengers side when looking diredctly at it

no, where the cam mark is, is not determined by the cam being 90 degree's to the head it is 42 teeth from the crank mark to the cam mark (as per holden workshop manual)
most aftermarket and all genuine belts have the marks already on the belts just count the teeth to see which way it goes on (this is a way more accurate way of setting cam timing instead of the backyard way
