while i was driving the steering arm came out of the rack (p/s) i nearly hit a pole, what would have caused this and how easy is it to fix? does it require a new rack? nothing seems to be broken, just unscrewed but it looks tricky to put back together has anything like this happened to anyone else?

What has happened is the rack knuckle has unscrewed, ive never in all my time seen that happen, id say that somebody hasnt dont it up properly. I would strongly suggest you speak to a mechanic or steering place about it and get the other side checked as well.

Has it had a wheel alignment recently. Undo the arm from the ball joint. Feed it back into the hole and turn clockwise until you feel it starting into the thread. Don't wind it in too far, maybe 25mm. Then reconnect it to your ball joint. This will allow you to drive it to a tyre shop to get an inspection and alignment.
